Found dead in her cryo chamber as reported in Mission Log "Day 25: [Isaac Higgins // 541-P] ". Her cause of death was thought to be the virus 541-P.
In Mission Log "Day 37: [Noah Dressler // Really bad jokes]", we learn that during the ship's fall through slipspace, an anomalous spacial disruption occurred, which "messed up a bunch of systems" and "poisoned her when the anomaly hit".
In Mission Log "Day 37: [Noah Dressler // Tactics]", it was discovered that the virus was a lie that Sacagawea perpetrated.
This classified information was collected from the Stonewall Maze in bits of Base64 code.

Alexis Ward, [something] expedition, final personal log, pre-cryofreeze. I'm sitting in my small office on my ship, looking out into the endless void of undiscovered, long-forgotten universe. Hard to imagine that this galaxy was once thriving with life, with a melting-pot culture so advanced and numerous, and yet they ultimately created the conditions of their own demise. Now here I am, a single human woman, about to set out on an expedition to find and study that very culture, but on a planet so distant, the distance is even one of speculation and hearsay.
I can only imagine what it was like there. Surely the culture on that planet, the life, the politics, their religion - all must differ so much from what little we know of the great Forerunner civilization. How truly far-reaching was their influence? How did they survive for so long and grow such an astounding civilization without destroying each other? I accepted this expedition so I can have the opportunity to be the first to gaze at a virtually unknown people. And during mission briefing from fact-finding, theories, our entire plan so compelling and exciting - I couldn't pass up this opportunity. She said she chose me not just because of my medical background but because of my aptitude and psychology of human and AI.
For this mission, we needed an AI that showed as much [?]experience in the field of scientific exploration as each one of the crew. With my connections and background in CIM, she delegated to me the responsibility of an AI for the ship. But with her limited monetary resources, she could only afford a crew of four, and that greatly limited the selection of civilian smart AIs. I had to resort to a cheaper [business?]. So I truly hope my selection of Sacajawea will prove beneficial. I have spoken with her to a great extent and she appears stable and healthy, given the coroner's [something] CIM class and her [something].
In recognition of her namesake, we have decided to call this ship the 'Lewis & Clark.' Higgins and Dressler like it. With this log, we have left Earth orbit and every preparation, [something] and diagnostic under the sun, checked and rechecked our navigational systems, coordinates, and all essential hardware. Every one is in excellent shape mentally and physically. We have said our farewells.
We have now retired into our personal chambers to prepare for cryostasis. It's going to be a long trip, even for slipspace. Sacajawea has promised a safe and relaxing trip. We're in her [realm?] now. This will be my final log before arrival. Go with peace, Essy.
